| 内容記述 | 本研究は，東日本大震災による液状化現象を受けた関東圏内にある子どもと家族が受診する一次医療機関がどのような影響を受けたのか，震災当日から通常の診察ができるまでどのような情報収集や情報発信を行ったのかについて現状と課題を明らかにすることを目的とし，医師1名，看護師4名にフォーカス・グループインタビューを用いた質的研究を行った． その結果，上下水道が停止しても「水があれば診療はできる」一方，「いつものようには受け入れることができない」現状であった．震災3日目，看護師は休診の情報を「先ずは留守電に入れる」「出入り口に張り紙をする」「救急隊への連絡」をしていた．ガスや電気のライフラインの停止により通信手段が遮断された際の連携の在り方が課題である． 震災10日目以降，幼児期の子どもは「紙おむつを着用せざるを得ない」状況となっていた． 震災1か月後「患者がいない期」とは，「ここでは育てられない」ことから子どもと家族は避難していた． 看護師は診察場面や電話相談での母親との情報交換を通して，液状化現象や計画停電の生活の場で起きている子どもの身体症状と家族の状態を見定めながら状況を予測し，そこから健康問題を捉えてかかわることが必要である． |

| 内容記述 | This study concerns a primary medical institution located in the Kanto area, attended by children and accompanying families, affected by liquefaction following the Great East Japan Earthquake. The aim was to identify how the institution was affected and to identify the efforts and issues of information collection and transmission carried out by medical doctors and nurses prior to recommencing their regular duties. Qualitative research was conducted using focus group interviews with a medical doctor and four nurses. The study found that even when the water supply and sewage stopped, medical care could be provided as long as water was available. However patients could not be accepted in the normal way. On the third day of the disaster, to notify of closure, the nurses first left a message on answering machines, posted notices at the entrance, and informed rescue crews of the situation. Cooperation between doctors/nurses and rescue crews is a key issue when communication routes are shut down due to lifeline failures. From the tenth day, toilet-trained preschool children had to use disposable diapers. The study revealed that one month after the disaster, dubbed the 'period of patients' non-existence', families left the community because it had become impossible to raise children there. When children are caught up in such situations, nurses need to determine, from medical examinations they attend and from talking with mothers who call for telephone consultations, the physical condition of children as well as the families' situation, and nurses should be able to predict future conditions so that they can anticipate and prepare for expected health problems. |
