| 内容記述 | 近年，急性期病院の一般病棟において，認知症高齢者が認知症以外の疾患治療の目的で入院する機会が増え，認知症高齢者のケアや治療を行っていく上での看護師の困難が増えている．本研究では，看護師の体験から，一般病棟における認知症高齢者への対応の困難さについて明らかにする．2病院の看護師5名に面接を行った結果，次の困難な状況が浮き彫りになった．；1. 予防策を実施しても生ずる事故，2. 認知症高齢者を尊重した対応ができない，3. 患者に安寧な状態を提供することへの医師の理解の得られにくさ，4. 家族に状況を理解してもらえないため長引く退院，である． 一般病棟は，緊急度の高い患者への対応が必要とされる中，人員不足により多忙な環境である．そのような中で認知症高齢者の管理をせざるをえない状況は，認知症高齢者の安全や人権を脅かしていた．また，それは看護師の負担感の要因となっていた．認知症の状況を受け止められていない家族に対しては，看護師の家族への理解が重要である．また，医療スタッフの教育体制や適切な人員配置を検討していく必要があること，地域でも認知症高齢者を介護している家族の支援を強化していく必要があることが示唆された． |

| 内容記述 | In recent years, on general wards at acute care hospitals, there has been a growing number of cases where elderly people with dementia are hospitalized for treatment of a condition other than dementia. Accompanying this trend, nurses are having increased difficulty in providing care and treatment for elderly people with dementia. This research clarifies the difficulties that nurses have in dealing with such elderly people with dementia on a general ward from the experiences of the nurses. Interviews with five nurses at two hospitals revealed the following difficult situations: 1. Even when preventive measures are taken, accidents occur; 2. It is not possible to provide care in a respectful manner; 3. It is difficult to gain the understanding of doctors toward providing peaceful conditions for patients; and 4. Because the family would not understand the situation, discharge from the hospital gets delayed. In circumstances where patients need urgent care and there is a shortage of workers, management of elderly people with dementia was a factor that threatened the safety and human rights of such patients, something that troubled the nurses. When a family does not accept the fact of dementia, it is important for the nurse to be understanding toward the family. The research also suggested the need to review the system for training medical staff and what constitutes appropriate staff assignments, along with the need to strengthen support in the community for families who are taking care of elderly people with dementia. |
