内容記述 Currently, most people meet the end of their own lives in hospital. Although palliative care units increase in number, many people still die in general wards. Therefore, it is necessary to enhance the terminal care in general wards. The purpose of this study is to reveal what and how nurses involved in terminal care see and think to improve the quality of terminal care in general wards. As a means to achieve this purpose, three nurses in charge of terminal care in general wards were interviewed one by one for an hour each in the form of a semi-structured interview.
The findings are summarized as follow. The three nurses thought it difficult for nurses to share sufficient information on patients' and have constructive dialogues with medical doctors. They hope to achieve the wishes of patients and their families so that the patients can complete their lives according to their will. But, the nurses also felt it hard to really understand those wishes, because what is disclosed to patients on seriousness of their disease conditions is often expressed ambiguously, because nurses do not have sufficient communication skills, and because general wards do not have an environment for nurses to take time to listen to patients due to lack in privacy and nurses' busy work.
As the result, the nurses thought they had missed opportunities to provide the patients with enough care.
The following necessities to enhance the quality of terminal care in general wards were found in this study. 1) To have proper records on patients to share detail information in both inpatient and outpatient sections and to establish well-computerized information systems to comprehensively review the records. 2) To facilitate reciprocal feedbacks on nurses’ care across the lines of work. 3) To have opportunities for face-to-face discussions among nurses, medical doctors and other co-medicals.
