| 内容記述 | The actual conditions of seating care for elderly persons using a wheelchair with decreased Activities of Daily Living (hereafter ADL) in a convalescence rehabilitation ward and their causes were clearly identified by interviewing four caregivers (a physical therapist, occupational therapist, nurse, and care worker). As a result, nine codes, “inclined sitting,” “sacral sitting,” “sitting correction,” “maintenance of the sitting position,” “assistance for activity,” “wheelchair performance,” “procurement of wheelchair and seating tools,” “seating process,” and “effect of seating,” could be abstracted. According to the care providers, inclined sitting is often caused by a wheelchair too large for the patient, but can also be caused by the paralyzed side being pressed on the seat due to not understanding sensory disorder and motor paralysis of a stroke. Sacral sitting is caused by decreased body strength and fatigue. Inclined sitting can be improved by inserting a cushion on the inclining side and using a wheelchair table, and sacral sitting can be improved by inserting a cushion under the knee to prevent the pelvis from slipping, or behind the back where it contacts the chair in order to ease the pain. Sitting correction, at one hour intervals, is considered to be essential for elderly persons with decreased ADL living in wheelchair. To get out of bed during the day and stay awake, helping the patient recognize the purpose of sitting in the wheelchair, and pursuing activities appropriate for each patient is considered to be important. For the seating process, the research subjects mentioned how they communicated together during work regarding the patients targeted for seating and the seating period at the hospital. In their everyday relations with the patients, the physical and occupational therapists recognized that the effect of seating influences the improvement of daily activities of the patient more than the nurse and care worker. |
