@article{oai:jrccn.repo.nii.ac.jp:00000031, author = {田口, 実里}, issue = {25}, journal = {日本赤十字看護大学紀要, Bulletin of the Japanese Red Cross College of Nursing}, month = {Mar}, note = {application/pdf, 目 的  本研究の目的は,感染管理認定看護師が資格を取得後に,役割・職位・給与の変化の現状を明らかにすることである. 方 法  質問紙調査.質問紙の配布数は85部で,回収率は67.1%であった. 結 果  感染管理認定看護師は,資格を取得後,79%が兼任または専従にて感染管理業務に携わる役割に変化していた.また,感染管理活動時間の増加とともに,院内全体を統括する役割を担っていた.自身の活動については,84%が上司に成果報告を行っているが,給与への変化は28%のみであった.個人的課題としては,活動時間の確保と活動成果に対する給与補償を望んでおり,組織的課題としては,感染管理システムの構築と強化,他部門との連携強化が挙げられた. 考 察 2010年の新療報酬改定に伴い,専従で感染管理活動を行うようになった一方で,兼務では,活動時間を確保することが難しく,看護管理者の理解が重要と考える.また,活動成果を報告するが,十分な対価として反映しないことから,モチベーションの維持が難しく,活動時間と実績評価を反映した給与制度の検討も重要であると考える, Purpose This study investigated how the roles, positions, and wages of nurses changed after receiving accreditation as certified nurses in infection control (CNICs) Method A questionnaire survey. The distribution of the question paper was Part 85, and the recovery was 67.1%. Results After receiving accreditation as CNICs, 79% of respondents began to work in roles involving infection control (IC) either exclusively or in combination with other tasks. As the time devoted to IC activities increased, they also assumed roles overseeing the hospital as a whole. While 84% reported the achievements in their own activities to their superiors, only 28% were rewarded with a change in wages. Regarding individual-level problems, the nurses reported a desire for sufficient time to engage in activities and wage compensation consistent with the results of their activities. Organizational roblems included the need for the establishment and strengthening of IC systems and the facilitation of collaboration with other departments. Discussion While the 2010 amendment to the medical fee system enabled nurses to specialize exclusively in IC activities, those engaged in IC in combination with other tasks encounter difficulty in securing the time to engage in these activities. Therefore, these nurses require the understanding of nursing managers with regard to this situation. Although the results of these activities are reported to managers, their responsibilities are not adequately reflected in wages, which can have a negative effect on motivation.
